The Southeast Quality Innovation Network-Quality Improvement Organization (QIN-QIO) is operated by Health Quality Innovators under contract with the Centers for Medicare & Medicaid Services (CMS). Our team delivers no-cost quality improvement services to nursing homes, hospitals and physician practices across the Southeast to improve health outcomes, lower costs and reduce provider burden.

The resources and support provided through the HQI's program have allowed us to keep our quality work moving in the right direction despite all the staff turn-over.
Our quality improvement consultant was extremely helpful in explaining the procedure and steps that were needed along the way. As I had recently assumed the infection prevention role at the facility, her guidance made the transition so much smoother.
HQI’s program sharpened our focused and strengthened our ability to deliver innovative approaches to patient-centered care. With support from our HQI quality improvement advisor, we’ve used data to drive change and exceed national and network benchmarks while continuing to be sustainable and patient-centered.
The project was successful in decreasing sepsis admissions and mortality and demonstrates the importance of collaboration between hospitals and nursing homes. This project should not only be duplicated but should also be considered for spread.
